Under-rated and off the beaten path, eh? Well, Colombia would be on my list, but you don't want to hear about that, so I'd say Nicaragua.
Nicaragua is very like Costa Rica in some terms: Same kind of rugged Pacific coastline with lots of great surfing opportunities, same kind of volcanic mountains crossing its core, same kind of rain forests for your jungle trek. But of course Nicaragua is very different too and shouldn't be expected to be a clone of any other country.
If you go, some of the highlights might include:
* Granada
* Islas de Omotepe
* San Juan del Sur
* Esteli
You could do a river trek across Nicaragua, down the San Juan river, or hang out on the beaches. Masaya (near Granada and Managua) is a great place to experience a live volcano, and nearby towns have great folk art souvenir shopping potential. Best of all, Nicaragua is a VERY cheap country to visit.
