visa is ok but it only lets you stay in the US for a certain period of times, after that you either have to apply for citizenship before the times expire or have some kinds of permission to stay. I don't know where you would get a passport in AZ since i've been there but you can check out uscis.gov and they will have information on where you can obtain or apply for a passport.

You can get a passport application at most US post offices. Some post offices are even set up to do your pic too. Then it's mailed in and you have to wait for approval. Sometimes several months, so make plans for it before you travel.
Whenever you leave the US for any reason, you should carry a passport. Especially nowadays with the terrorist situation in the world.

You say you live in Arizona, but you don't say if you are a citizen of the USA. If you are a citizen of the USA, then you need a passport-approved photograph (most photographers, or Walgreens can take them). Take that photograph to the the main US Post Office in your town, and fill out some paperwork. There is a fee (it used to be $25, but I think it's more since Homeland Security was started).
If you are not a US Resident, then you can't get a passport where you are. You will need to return to your country of citizenship and get one issued there, apply for a visa, and come back to the USA through legal channels.
