I think it´s about €120 for an individual membership, and €300 for a family,per year. They offer health checks, but possibly more reassuring is that they will come out to you in all of AndalucĂa in either ambulance or helicopter, always have an English speaking doctor at the scene and will repatriate you if you´d like.
