Wait a minute, some of these answers are wrong.
If you are working legally for a Spanish company and therefore have a nomina (payslip) then you will get free health care as any Spanish subject would as you are treated the same. Your family can be registered on your Social Security number too (I am registered on My son in laws so I don't pay)
If you are employed by a Spanish company then the documents from the UK are invalid as you are earning and paying tax to the Spanish government so therefore entitled to the benefits of health care.

This is a complicated question... you're from the U.K ? You'll need the ECH 11 ?? or something then you can get free health, but if you're over 65 you can also get free health if you become a citizen

But for example if you break your leg you can get it free here, but if you get Cancer and need some special drugs oftern you'll need private insurance because the E.U Won't pay for these

In my opnion it is a hard choice, life or making your family poor to pay to keep you alive

P.d my cousins boyfriend is from Romania, and he broke his leg and he got it all free, my sister broke her leg and she had to pay for most thing, I think it depends on the Hosptial and your luck
INSALUD is the free health care system,
http://www.seg-social.es/Internet_4/index.htm?ssUserText=
it comes from your tax you pay when you work

In order to get Spanish state health care you need to pay into the Spanish social security system. If you have a proper contract with an employer your employer pays your contributions. If you're self employed you pay your own contributions as an 'autonomo' (presently nearly 250 euros per month). Your husband/wife and children are covered if you pay into the system. Having a residencia has got nothing to do with it. There's no obligation to get a residencia and you can live here for years without one if you so wish. You'll need an NIE number though.

first thing you need is an nie number you get a social security card after a 6 month qualifying period the dwp in the uk will issue you with a form e121 at the moment there are so many uk expates tyring to get health care on there European health cards that witch are for emergency only that they are asking for proof of travel ie plane tickets or they will not treat them. once you are in the system here after the qualifing pereod you relinkquish all rights to medical care in the uk if you return to the uk you have to qualify for treament again by paying into the system for 6 months

Yes if you come back to Britain for it. Will you continue to pay your National insurance for this care in Spain?

Snakeman !
ONLY if (a) you return to the UK for treatment or (b) Spain has a 'national health service' !!

You'll be entitled to free emergency health card with your e111 but nothing else. If you intend on staying permanently in Spain, get your residencia, then there is a qualifying period before you gain your social security number. However, you will have to make contributions before you qualify.
Many Brits move to Spain and don't tell the NHS they are going and return home for treatment, but you are supposed to tell the NHS before you leave.
The company you work for in Spain should assist you with all relevant information.

Quite simply No you wont, you should take out healthcare upon your arrival in Spain. Some employers there do provide a scheme to contribute into.

Yes. You work and pay your taxes like anyone else, you get your Social Security number and you're entitled to free health care like anyone else. Not sure about your family.
