Malpensa is quite a way outside the city of Milan (which is a madhouse). I assume they would have a train. Fly to Zurich and then drive or take the train to Bern. Do you have a car? It depends on how much time you have, etc. There is really only one road from Milan into Switzerland through the Gotthard and then North around Zurich and then to Bern. This is a long, tedious drive depending on traffic. It will take you at least 5 hours. There is another route to the West, but that is even more tedious. Look at a map. It is very easy.
