They can file a report on your credit report. They can send you to a US collection agency who can sue you in small claims court. But you should pay, because you went to them for treatment and they are not obligated to give you free medical care. Next time, get travel insurance. Ask your insurance company, again, to pay. First try calling your insurance co. Try to find out why the insurance co. won't pay. Sometimes, it's bc the European hospital will not itemize the bill (sometimes, they will just bill for an amount with no explanation). The hospital/dr. might get paid if they itemize the bill. Then try writing to them (insurance co). If your insurance is through work, ask your human resources dept (personnel office) for help/suggestions.

If you have health insurance in the US, they will pay the bill. Tell the hospital to itemize the bill (in English if possible), or tell your insurance to deal with them directly. (unless you have Medicare, which does not cover overseas medical expenses)
If you don't, they can file a lawsuit in the US. US and Switzerland have bi-lateral agreements. Don't be fool, thinking they cannot do anything.
