In a sentence, usually but not necessarily after the name of a town/city it would be "Md." (small "d" and a period...even if it's in the middle of the sentence).
On an envelope it would be "MD" (both capital letters & no periods).
"If you ever visit Md. it would be a good idea to visit Baltimore, Md. after writing to the Visitors Bureau at 333 Wikmore St, Baltimore, MD 11122." (not a real address).
