The chances of getting a Vietnam registered scooter passed the border are next to Nil. Yes you will see lots VN numbers on the Cambodia side near the border but how they do it, I just don't know.

You will need your passport, ownership papers proving the motorbike belongs to you, Cambodian visa and your Vietnam entry / exit card.
Do NOT bring a lot of cash with you because it IS a very dangerous trek. More so in Cambodia than in Vietnam.
